Kalexedy Learning Center: Terms and Conditions

I hereby waive and release Kalexedy Learning Center, LLC (KLC). the staff, volunteers, and any other persons associated with this event of all responsibility and liability of any nature whatsoever as it concerns any/all injuries, sickness, or damages incurred from my child's participation. I am aware that the activities involved are sometimes of a physical nature and therefore accidents and injury may occur. I consent to the staff of KLC administering first aid and contacting emergency services immediately, should the need arise. I also give permission to KLC and its authorized representatives to arrange transportation for emergency medical treatments or tests. As the participant(s)'s parent/guardian, I agree to release any medical records necessary for treatment, referral, billing, or insurance purposes. Further, I certify that my child(ren) is/are physically fit and capable of participating in the activities for which he/she/they are registered. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the camp to secure and administer treatment, including hospitalization, for the participant(s) named herein. This form may be printed/photocopied for providing to medical providers out of KLC activities.

I give my permission for the free and unrestricted use of the participant(s)'s picture in any telecast, broadcast, or written account of this program. I give concent for the participant(s) to be photographed or videotaped during attendance at KLC and release the use of images by KLC for promotional, marketing, or other business purposes. These images are the sole property of KLC for use at their sole discretion. If registered participant(s) do/does not wish to be photographed or included in any photograph, they may notify KLC in writing or simply step away from the camera.

I understand that I am responsible for completing all KLC required paperwork before the participant(s) attends program(s) at KLC.

I understand I am responsible for payments for all programs registered regardless of whether the registered participant(s) attends. A non- refundable deposit of $75.00 per registered session per participant is due at the time of enrollment. This will be applied to the session fee with the balance due by May 15th, 2025 or one-week before the first day of the session or whichever comes first. Fees may be paid by ACH withdrawal, credit card, check, or money order. Checks and money orders must be payable to "Kalexedy Learning Center". A $50.00 returned check fee will be assessed for checks returned as uncollectable (NSF). KLC reserves the right to cancel any session. If KLC cancels a session, all deposits and session fees will be refunded.

I acknowledge and understand that Kalexedy Learning Center, LLC has established rules and regulations pertaining to conduct, behavior, and activities of all participants. Participant(s) and I agree to abide by these rules and regulations during attendance and that Participant(s) and/I will be responsible for his/ her/their/my failure to abide. Participant(s) and I understand that violation of the rules
can result in dismissal with no refund.
